We're seeking a compassionate Hospice LPN to join our team and provide skilled nursing care to patients in the comfort of their own homes. This is an opportunity to offer vital support and make a meaningful impact on patients and their loved ones.
Your Rewarding Role:
Provide Compassionate Care: Administer skilled nursing care under the supervision of a Registered Nurse, ensuring patients receive the highest quality of care.
Collaborate with a Team: Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
Support Patient Needs: Assist in identifying patients' physical, psychosocial, and environmental needs, and participate in planning and implementing care.
Ensure Continuity of Care: Provide effective communication to patients, families, team members, and other healthcare professionals.
Maintain Quality Standards: Monitor assigned cases to ensure compliance with requirements and promote agency philosophy and policies.
What You'll Bring:
Education: Graduate of an accredited school of vocational nursing.
Experience: One year of experience as a Licensed Practical Nurse in a clinical care setting (home health experience preferred).
Skills: Nursing skills as defined by generally accepted standards of practice, and good interpersonal skills.
Transportation: Reliable transportation for travel to patient homes.
Certification: Valid LPN license.
